The following information about our facility will be valuable in familiarizing you with our services and procedures. Please read it carefully.
The Business Office is open Sunday through Friday, from 9:00 a.m. to 5:00 p.m. We would appreciate payment of bills during these hours. All charges are payable weekly, in advance. Any additional expenses will be billed separately. Information regarding application for Medicare or Medicaid will be provided upon request.
Hours are 8:00 a.m. to 8:00 p.m. daily. (Suggested visiting hours, 11:00 a.m. to 8:00 p.m.) No visitors are permitted in the dining room during meals. Please cooperate by not visiting if you have signs or symptoms of cold or flu. Children are welcome when under the supervision of an adult.
Breakfast – between 7:30 a.m. and 8:30 a.m.
Lunch – between 12:00 noon and 1:00 p.m.
Supper – between 5:00 p.m. and 6:00 p.m.
Afternoon and bedtime snacks are served at 3:15 and 8:30 p.m.
Requests for information about a resident should be made between 9:00 a.m. and 8:00 p.m., if possible. Private telephones may be installed. Calls to residents should be made through private telephones or the public telephones. First Floor – 908-355-9793; Second Floor – 908-353-9727. Please avoid calls during meals.
LEAVING THE PREMISES
A signed release is required before a resident leaves the premises. Please see nurse in charge. Residents should return by 8:00 p.m. Advance notice must be given if medication must accompany the resident.
SAFETY
The following rules, as requested by the Fire Marshall, are for the safety of residents, visitors and staff:
1. No smoking in rooms at any time by visitors or residents.
2. Cigars, cigarettes and matches for residents should be left with the nurse in charge.
3. Smoking for residents in designated areas only!
CHANGE OF ROOM
Prior to a change in room, the facility will notify the resident and, if known, the resident’s legal representative or interested family member and provide an explanation for the change.
PERSONAL CLOTHING AND LAUNDRY
Elizabeth Nursing and Rehab Center is not responsible for personal clothing. Residents should have on hand a one-week supply of clean, comfortable clothing. Each article of clothing must be marked with the resident’s name for identification. A laundry marker may be borrowed for this purpose.
Arrangements can be made for laundering of personal clothing. Soiled clothing cannot be kept more than one week. This facility does not assume responsibility for personal articles lost or stolen. Clothing should be checked periodically for mending, and names re-labeled with laundry marker.
FOOD
Elizabeth Nursing and Rehab Center operates a kosher kitchen and serves kosher food only. Appropriateness of food brought in for residents should be checked with nurse in charge. Food left for residents should be in containers with tight fitting lids. Please do not bring food that must be refrigerated.
DENTURES, HEARING AIDS, AND EYEGLASSES
Reasonable measures will be taken to safeguard dentures, hearing aids, and eyeglasses. However, ultimately the facility cannot be responsible for loss of same. These should be marked with resident’s name for identification, on admission. Dental exam by facility dentist or written documentation of exam must be provided within 180 days of admission, in accordance with State requirements.
When patient desires to retain his or her family physician, the resident or his representative is responsible for arranging the initial visit. The State of New Jersey requires all residents to be seen within forty-eight hours of admission.
All Medications will be ordered by the nurse in charge, from Specialty Rx Pharmacy or another registered pharmacy, and billed to the resident or representative. According to State regulations, no medications or medicinal type items may be kept in resident rooms. All such items will be removed. State laws also prohibit pharmacies to take back unused medications, once dispensed.
It is against the policy of Elizabeth Nursing and Rehab Center for employees to accept gifts or tips from a resident, family, or business concern.
A full program of planned activities, including provision for religious needs, is conducted for all residents. A monthly calendar is posted on each floor. Families are encouraged to participate whenever possible.
Licensed barber and beautician services are available regularly on a direct payment basis.
The nurse in charge of the floor/unit should be the first person consulted if there is a problem. If the problem is not satisfactorily resolved, the Director of Nursing or Administrator should be contacted. State survey is available in main floor lobby.
Residents may bring with them items such as a television, radio, picture, painting, plant, or favorite chair. Please inform the business office to check regarding suitability or space. We suggest that residents not keep cash, jewelry and other valuables in their possession. These items may be kept in a safe available in the business office on the main floor. The facility does not assume responsibility for personal articles lost or stolen.
NOTE: Personal Needs Allowance, (PNA), can be withdrawn from Sunday through Friday, between 9:00 a.m. and 5:00 p.m.
In case PNA funds are needed for Saturday, please notify business office Friday.
Elizabeth Nursing and Rehab Center maintains an active volunteer program open to relatives, friends, and members of the community, For further information, please contact Activities Director.
RESTROOMS
Restroom facilities for visitors are available on the main floor only.
FAMILY GATHERINGS/PARTIES
The main floor Recreation Lounge may be reserved for private parties, (no charge), by contacting the Business Office. Sponsor must supply any necessary utensils and/or disposable supplies.
